Important Info

When To Use These Forms:

Activity Form:

• Must be filled out for EVERY Troop activity we do outside of a Troop Meeting (i.e. weekend campouts, beach cleanup, serving as color guard at veteran's ceremony)

• Think of this as a school permission slip - if you don't have it, you can't attend!

Medical Forms:

• Forms A, B1 and B2 - must be filled out by Scout's Parents (or Adult Leader that will be camping over 72 hours)

• Form C - must be filled out by Scout's Doctor (or Adult Leader's doctor)

Medicine Permission Form:

• Must be filled out by Scout's Parents each year and placed in medical file
